16 week log, shoulder that had not improved in two years

Said I would post this so here it is. Same shoulder from my panicking-about-my-first-injection thread in March. Setup. 31, 84 kg, left AC joint, originally hurt in a snatch in November 2024 and never properly resolved. Pain on overhead pressing, pain sleeping on that side, fine for most horizontal pulling. Protocol. BPC-157, 250 mcg twice daily, 5 mg vial in 2 ml so 10 units per dose on a 100 unit syringe. Subcutaneous, alternating between the outer delt on that side and the abdomen, no particular logic to which. Ran it 8 weeks on, 3 weeks off, then 5 weeks back on. Total 16 weeks elapsed. Alongside it, and this matters: physio twice a month, a proper loading programme built around banded external rotation and slow eccentric work on landmine presses, and I stopped snatching entirely for the whole period. Numbers. Week 0: overhead pressing pain 6/10, resting 3/10, sleeping on that side impossible, could not press an empty bar overhead without wincing. Week 4: overhead 5/10, resting 2/10. Sleeping slightly better. Honestly could have been the deload. Week 8: overhead 3/10, resting 0 to 1/10. First night I slept on that side by accident and only noticed in the morning, which was a genuinely good moment. Week 11 (end of the off period): held. Did not regress. This is the part I found most interesting. Week 16: overhead 1 to 2/10 and I am pressing 45 kg for sets of 5, which is not impressive but it is 45 kg more than I could do in February. What I cannot tell you is how much of this is the peptide. I changed three things at once. A stubborn injury plus a proper loading programme plus removing the provoking movement is a combination that fixes a lot of shoulders on its own, and I know that. The thing that makes me think there is something in it is the rate, this thing had been stable at 6/10 for over a year with physio already happening, and it moved. Side effects: none I noticed. One injection site that got a small red lump for about two days in week 6, went away on its own. Cost, since people ask and nobody says: roughly R2 400 across the whole run including bac water and syringes.

The week 11 detail is the most useful part of this. Holding through a three week off period is more informative than the improvement itself, because if it were purely the deload and the movement removal you would expect the trend to keep going or plateau, not specifically hold. Also thank you for putting the rand figure in. Everyone posts logs and nobody posts costs.

Good log. One gentle pushback on the interpretation though. You had already been doing physio for a while, but you started a proper loading programme at week 0 of this run. Those are different things. "Physio twice a month" for a year could have been mostly manual work and advice, whereas structured progressive loading is the intervention with the strongest evidence base for exactly this kind of problem. So the confound is bigger than it looks. Not saying the BPC did nothing. Saying that if I had to bet on which of your three changes moved the number most, I would bet on the loading programme, and I think you would too if it were someone else's log.

Yeah that is fair and I think you are probably right. I would still run it again for the next thing though, which possibly says more about me than about the evidence.

Thanks for writing this up with numbers and an honest confound section. Logs like this are worth more to the forum than ten "it worked amazing" posts. For anyone reading it later: note that the baseline was documented from week 0. That is the whole reason this log is readable. Start your notes before you start your protocol.
